Registered Nursing
Assess the patient's condition to develop an individual care plan, reconcile medication, coordinating with other members of the home health team, updating your physician of health changes, teaching skills of care and disease process to family members and monitoring vital signs and health status, wound care - including dressing and cleaning wounds, performing injections and complex treatments, checking and helping with catheters and feeding tubes, provide IV therapy, anticoagulant monitoring, and psychiatric care.
Physical Therapy
Treatment to improve mobility, regain strength, balance, relieve pain and improve function after an injury, surgical procedure, or medical diagnosis. Evaluate the patient's condition to develop an individual care plan, reconcile medication, coordinating with other members of the home health team, updating your physician of health changes, teaching skills of care and disease process to family members and monitoring vital signs and health status, wound care - including dressing and cleaning wounds.
Occupational Therapy
Services to help you return to usual activities (like bathing, preparing meals, and housekeeping) after illness. Occupational therapist our experts in activities of daily living training, fine motor coordination, sensory treatment, orthotics, adaptive equipment, and assistive device training.
Speech Therapy
Treatment to improve communication problems involving speech, language, hearing, and swallowing. Or for the treatment of other upper aerodigestive disorders, also includes: swallow evaluation and treatment, voice disorder, speech articulation, dysphasia treatment, language disorder
Medical Social Work
Skilled services to help you with social and emotional concerns related to your illness including counseling or help in finding resources in your community. Medical social workers apply their knowledge of social and psychological theory for long term care planning, resource planning and coordination, assistance with advanced directives, senior housing application, financial assistance and counseling.
Home Health Aide Services
Part time or intermittent services to help you with your daily living activities including: personal care, activities of daily living, grooming and hygiene, assist with your use of ambulatory aids, range of motion exercise
